No, it's the same. Silikon 1000 is just a medical grade brand, but it's the same shit.

It was only ever FDA approved for fixing retinal issues. Some use it off label with some success in very, very tiny amounts in the face (true micro drop technique) to permanently fill divots/imperfections etc. Some use it in lips. And some monsters use it in large volumes in other body parts with the FDA warns against. It's the same crap you can get in a back alley for all intents and purposes.

So now I'm super confused because that is what he was using on guys on this forum, myself included. If it's the same I'm a little speechless that he is making a video about how bad it is for you. I'm sort of at a loss for words.

He can answer this for himself as well, but I actually asked Ian about silicone when I was there. Mainly because I was curious about the original smooth procedure they offered with Silikon (prior to now using just PMMA in small quantities). I also was curious about it because of the whole scrotal fill thing (not saying I want it, but I was curious).

The situation I was told was that they had one (singular, one) issue with a reaction from it out of all the guys they've done. I'm not sure if it was serious enough and/or the guy was upset about it enough that it caused them to really re-evaluate if it was worth using at all.

I asked about how many serious complications they've had with Silikon 1000, how many they've had with PMMA, and how many overall they figured they've done over the years. The complication rate serious enough to warrant Removal with PMMA from their clinic was extremely low (and from what I recall, the Removal case(s) were uncircumcised and it was more due to issues related to appearance than scary stuff). So if their rate of complications is so low sticking only with the collagen building fillers, why after having one bad outcome with a silicone patient would they bother keeping that added risk in the mix?

I honestly wouldn't be too worried if I were you, I mean how many CC of it did they use? The documented complication rate with it used in tiny amounts for microdrops is low. It's once you get anyone lying about how it's being used. Pumping a Dick full with 20-30-40-50 CC of silicone at a time is not micro drops irregardless of what someone says, and it's not the same as using it how it was intended.

I'm not really sure how much was used, but probably more than most guys on her. While my Dick looks very natural Erect, it looks bizarre Flaccid. I won't take my pants off at the gym to change. PArt of it is the silicone which has nowhere to go when Flaccid so it bunches/puddles up until my Dick gets Erect. Sometimes you just need to let go of these things once they are done.

I think stuff like that was why they started using it in small amounts to begin with, to try and fix little issues that I'd assume some guys are really pushy about and can't be filled easily with PMMA. Which sometimes would, and sometimes wouldn't. Any kind of cosmetic thing, people are limited by their own canvas and some won't accept that I'd say but that's just my opinion on a broad issue that's really unique to every one.

I'm not pushy, it wasn't about that. I'd say about 2 years now - it's Ok - just going to let this go as there is nothing to do about it now. But I am surprised by the video given that they were offering the procedure for a year or 2 on this site and then make a video saying how bad it is for you.

I've spoken to him personally about this, and well before this video was made. I'll let him chime in if he wishes, but this is what I recall from our conversation.

Silikon 1000 is FDA approved via micro-droplet injection technique --> that is to say, very low volume, done in micro-droplets so as to suspend the silicone oil and some trace collagen, often done superficially. This made it an ideal candidate to fill in problem areas, especially where the skin was less compliant or accessible (think Circumcision line area, behind glans, etc). In theory, and per FDA approval for micro-droplet use, they went ahead and used this in conjunction with PMMA to provide a more natural, aesthetic result. I've seen some of the Silikon 1000 work and they definitely turned semi-suspect units into very natural looking ones. This distinction is important because, although he's outright discontinued silicone, much of what he explains in the video pertains more to large volume injections of silicone (he even says in the video "especially if large doses are used"). Furthermore, despite advancements in Phalloplasty, it still remains largely experimental. I'm personally glad he has the professionalism to recognize an issue rather than double down. He once employed the use of [forgetting name of filler, will edit later.] but later discontinued it due to unacceptable complication rates that weren't apparent during trials [Reference Edit]. Phalloplasty historically (and even currently) remains largely experimental & evolving, we just have to hope that the top names in the field are willing to adapt & refine where and when necessary.

Then one day he had a patient that had a fierce reaction to the product. Whether Silikon 1000 was the direct cause didn't matter, because the domino effect of complications that were exacerbated by the silicone oil made him reconsider its use. In all his years injecting PMMA, he had never had a severity of complication in the manner of the silicone patient. After some review and consideration, he decided to discontinue its use all together. That isn't to say past patients have a lot to worry about, and I'll give my personal opinions as to why:

  • There is no clear evidence I'm aware of that the Silikon 1000 was the direct cause of the complications found in the aforementioned patient. Due to HIPAA and the own patient's limited reporting (which I don't blame, I hope he'll fill us more in when he's ready), there simply isn't a clear enough picture to put the blame entirely on the procedure itself. Suffice it to say, complication from silicone injections have always been known, albeit low, and probably much higher in much larger volumes.
  • While I personally don't have a lot of faith in the FDA, their approval gives some credence to the product having a much better safety profile when injected in very low volume.
  • Dr. C made this choice independently, from a medical & practical point of view. However, I would be remiss not to point out the No-Silicone-Zone's potential influence on the decision. The No-Silicone-Zone is my passion project this year that wont bring me in any income/benefits. It's a PSA regarding the dangers of silicone use for penis enlargement. This includes oils, implants, etc. This field lacks standardization, and many Physicians employ their own techniques, their own choice of fillers, etc. I figured a good way to get establish uniformity in this industry is to find a cause worth rallying behind. That's when I was able to get the No-Silicone-Zone endorsed by all my Sponsors (and some non-Sponsors pending as well). It effectively rejects & denounces the use of silicone products for cosmetic penile enhancement as both unethical and unsafe.

    Although one could argue micro-droplet technique using an FDA-approved Silikon 1000 (used in many thousands of patient's faces) can be the exception, I argued it would be wiser to be done with silicone altogether. This may or may not have played a role in Dr. C's decision moving forward, but I think it's important to know that a significant policy change of this sort doesn't necessarily imply or suggest that past recipients of Silikon 1000 are now at imminent or mortal danger of losing their penis skin in the near or distant future. Instead, I suspect a decision like this encompasses everything from science/medicine, patient welfare, public relations, liability, etc before concluding. In other words, policy-changes alone aren't always cause for alarm. These are just my observations & opinions, and Dr. C is certainly welcome to chime in if he wishes.

*If I'm not mistaken, assuming Dr. C is willing to disclose this, some of his employees (past and present) have been administered Silikon1000 injections for aesthetics, and furthermore, I've not heard of any major complication stemming from a Dr. C Silikon1000 outside the aforementioned patient above. I would be interested in knowing if there have been other reported cases.

So backs up all of what Ian said to me in late January then. I couldn't remember all the specifics, I was on ativan or whatever it's called and a little more preoccupied with my own day. He did say a couple times that it was *one* instance of issue.

Hello, DrC here.
For the longest time, I was an opponent to the use of silicone oil because of its known side effects related to the high volumes used for buttocks, breasts, muscles, etc.
During the epidemic of full-blown aids and facial lipoatrophy, several doctors in NY and LA published about the safety and effectiveness of a specific technique that preconized the off-label use of tiny drops (micro-droplets) of medical-grade silicone oil injected independently in small areas and low volume.
I was there, working with facial lipoatrophy patients since the early 2000s and amongst my PMMA patients, there were many that had been treated with the silicone microdroplet technique in the US; some of them still come to my office occasionally.
My observation of a large number of those patients and the scientific publications backing up the safety of silicone in microdroplets (Derek Jones MD and others) gave me the confidence to offer it for the smooth finish in Phalloplasty patients, and it proved safe again. There are other publications of safe results for the treatment of acne scars as well.
However, no medical procedure is 100% safe; an extremely low number of patients are intolerant to the product, and I learned about it the hard way: one patient presented a fierce reaction a short time after receiving a couple of sessions of it, and the treatment has been very difficult because (different from PMMA and PCL and HA) the product is not extractable.
One complicated patient is way too many, and I did not want to risk finding another person with the same predisposition. I removed it from my practice.
All of us that received the microdroplets back then is safe and no adverse reactions are expected since our bodies have accepted and encapsulated it with no problem.
The reason for the video published at the beginning of this thread is to help fight those practices that use the silicone oil not only as microdroplets but especially those that use it in large volumes: the risks skyrocket, and migration and deformity add up to the possibilities of complications.
Last but not least, my awareness pinpoints to the worst of all: the unethical use of silicone oil by practices that falsely offer it as "PMMA", "growth factors", "super-collagen", personal brands, and other names that give the idea of an ethical product, or no brands at all.
I hope that other practices and physicians endorse the PhalloBoards No-Silicone-Zone initiative.
