Question is whether or not you want a permanent solution or don\'t mind (and can afford) temporary solutions with yearly top-offs. Dr. E & Dr. L, let\'s be real, not good options.
If you want permanent, Dr. C in Tijuana or Dr. N in Prague have surfaced as good options. Dr. C has the most progress reporting patients, so you\'ll probably get the best overall impression of what to expect if you went with him.
Temporary filler doctors are more numerous, and there are a handful of names being mentioned (including the ones in this thread) who offer temporary filler enhancement. The only problem is that there isn\'t enough progress reports on these physicians as of yet to determine who the most skilled & successful is.
