As for the Penuma, you'll find that it's not a very popular option on the PhalloBoards as it has the most reports of complications or dissatisfaction among forum users dating back 13 years. I'm not surprised to hear they're interested in something that'll aid in preparation for the device given its propensity to fail (i.e. the silicone implant). Now as far as it not precluding fillers, is this true of all fillers? Be it Fat, PMMA, Hyaluronic Acid (HA), etc? Thanks.

I created P-Long because it is safe, it works, and it made my patients happy. It does not preclude any additional procedures like fillers or even the Penuma. I was speaking with one of the top Penuma surgeons the other day, and we agreed that P-Long before Penuma might be a great option because Penuma does not increase erect length, but P-Long does, so doing the P-Long before Penuma is the best of both worlds. If you are doing Penuma, the trans-scrotal approach is much better. Not that I am advocating for Penuma.

My guess is that you've got some scar tissue that's preventing your penis from reaching its full length. It's something you see with guys who've had the penuma removed. Some of them have reported success regaining lost length with stretching. Might be worth seeking out penuma/Elist removal reports on here.

A loss of two inches of length and an infection hospitalizing you for a week aren't things you typically see with PMMA. What doctor did you go to?

Dr. Loria is not a surgeon and therefore should not be performing any sort of procedure that involves general anesthesia and scalpels. More so, his recently introduced LEEP (sp?) procedure is basically an inverted Penuma implant and I can't begin to tell just how reckless it is in design alone. Reminder, this man is not a Urologist, not a Reconstructive Urologist, not an Andrologist, and not a Plastic Surgeon.

Lengthening procedures (also known as ligamentolysis) can be quite hit or miss. Much of it depends on your inner penile anatomy and some guys gain more than others. Flaccid length tends to get the most benefit, and the real challenge is following weeks/months of stretching in order to cement your lig gains.

There is a saying, you can't make everyone happy.

I could take a buddy visiting from out of town to my favorite restaurant, and insist he gets the ribeye steak. Upon arrival, the steak is overdone and clearly we're both disappointed. Of course they apologize and bring us out a new steak and free desert on the house. So even the finest restaurants occasionally butcher a steak. That's my analogy for the vast majority of quality physicians performing penis enlargement. The true can be said for a reputable barber or tattoo artist.

Dr. Loria on the other hand is the kind of establishment you go to crossing your fingers the chicken isn't days old and or whether or not the cooks actually earned the (fake) culinary degrees on the wall. Yea that's right, the guy said he was a Cosmetic Surgeon and frankly, he's not. However, his real expertise was Nutrition and Hair Restoration for some time.

Silicone-enhanced penises are what I consider the great deception in male phalloplasty. Results are instant, permanent, and often-times quite aesthetic. However, large volumes of silicone oil are not one of the safer alternatives in penis enlargement. Firstly, you're required to wear a tight band at the base of your penis just to prevent migration. That alone is a scary prospect. Then, if you happen to be one of the unlucky few who end up dealing with an unusually fierce reaction to the the oil, it becomes very difficult to remove, and can even require skin grafting. I know someone personally who is undergoing illness and it may be attributed to his obsession with silicone oil.

But more importantly, and I've read and seen this around a few times and it's such a valid consideration: if silicone oil is available off-label by the use of any of these phalloplasty doctors, why do the vast majority of them refuse to use it in the penis? Urologists, Plastic Surgeons, Dermatologists, all of them can use it, but instead they opt for Hyaluronic Acid (HA), PMMA, Ellanse, Renuva, Radiesse, and Fat. Why is that? It's because they don't want to deal with the mess that is silicone-enhanced penises.

And what about the PhalloBoards? You don't think I couldn't delete the negative comments and newcomers wouldn't be the wiser? I'm sure the likes of Loria would pay me handsomely considering how much he spends marketing a month. I genuinely care that my readership has access to pool of physicians that perform ethical medicine, it's how I built this site over a decade. I've turned down a number of Clinics I found to be shoddy or unscrupulous.

Also, when speaking to Doctors (both Sponsors and non-Sponsors alike), I'm told this silicone stuff rivals the Penuma in "repair" jobs on these poor men. If you really wish to enhance your size, seek out a vetted, experienced doctor who practices ethical medicine.

Oh and @EricPig , sent you a PM pretty much saying what I said here. I don't mean to alarm you as you may be just fine (not all silicone patients face complication), but there are many Clinics that will turn you away if you've had work done with Loria. Just an FYI, and be honest with them, last thing you want to do is encourage a complication by withholding necessary medical info. Good luck either way!

This forum should be a place where all of are here to encourage one another in the one thing that brought us here - feeling more confident in ourselves. It’s ok to have different opinions but everybody has different results and different experiences. You can’t say one particular place is bad…. All the companies pin each other against themselves.
Maybe having a more open mind and talking to multiple patients from all the companies (not just the ones promoted here) and seeing how they have faired. I know I have, and the majority of patients I’ve spoken too who have had blisters, pustules, vesicles… etc are guys who have had a lot of filler put in, regardless of the type. However, I don’t come on here blasting those companies that I’ve heard bad experiences from because I realize that every patient is different and reacts differently.
You’re quick to throw Loria under the bus and it’s ok if you choose not to work with them. But the issues being discussed in this forum are no different that issues that have been reported from companies you support like avanti derma. Would it be fair for me to reply under those threads that that company is bad? No. Because they aren’t. I know they do good work. However, you will have bad experience with every company. There’s so many variables. If you were a medical professional, which I can’t imagine you are with your narrow approach to this, you would understand better.
The point here is to please be mindful of your opinions since they are not authority, even if you feel passionate about them.

If I'm rude it's because silicone in the penis has a long track record of being much more dangerous than other fillers, fat, grafts, or really anything other than penuma. There are risks of infection, migration, and worse that never go away once it's put in. It creates large avascular areas where infections can grow. If your body can't get blood there, it can't fight an infection. I'm not wishing ill on you or anyone else who got it, I hope things work out for you. But this is a public forum and it's important that people reading this know how much riskier silicone is than other options, not to mention the unethical behavior of Loria in particular. You can see it in this thread; when things go south, he disappears.

No I didn't here about the procedure from lpsg. I found loria medical by Google. But I learned about lpsg when I called loria medical and had a phone consultation with BBone. Who just happens to work for loria. But yes I'm over there as Dred123. At that point I was schedule for a procedure when I learned about lpsg. Two and half years ago when I started my journey I was planning on getting the penuma implant from dr. Elist. That idea didn't last long because of the bad reviews I found on him. I kept on searching other enlargements and found loria medical which I couldn't find anything bad on him at the time. I read horror stories about penuma, fat transfer and pmma and other enlargements techniques but nothing on loria. So I decided to go with him. Its wasn't until about three weeks till procedure that I found some crap about loria which was on this site. I had everything already planned and still couldn't find any law suits on loria like I did on other doctors so I ended up going forward with it. (Which I have found out since then that Florida has laws protecting doctors in the state.) I had the procedure done and joined lpsg to share my story. Procedure 1 went prettywell and turned out great I was happy with the results and was the owner of a big penis. About 60days later I went back for another glans enlargement. That turned out great and now I had it all thick shaft and an large head. I was living it up and my confidence was sky high. So at that point I was schedule for another procedure in three months. I was very pleased and was planning on going from big dick to monster cock. Well procedure 2 started out as normal and I had a monster cock now. But about two weeks after procedure things went south. I got an opened wound on the rim of my head and my penis was really swollen. I had 8 3/4 girth limp. After two months of healing and no sex my penis was a monster disaster. It was really uneven bumps and dents, squishy when hard and three hard bulging spots with a scar on my head and when limp foreskin covering my head. (And yes I have been cut) It just looked horrible! Talked with loria and he said he would fix it for free but I had to wait. So five months later I go back to get it fixed and everything seems to be a little bit better after the procedure. Its getting firm again and the unevenness is better and the scar is half gone. So at day 25 I start getting an opened wound on my shaft under side next to my balls. They proscribe me some medication for it but it doesn't really help. Kept texting them what to do and then they give my the silence treatment couldn't get a response of what else to do. Finally on day 35 they responded back that it was day 35 and I can have sex again. Lol I still had an open wound at that time (no sex was happening I knew better) hell I had to take two weeks off work from day 25 and sit in my garage with my pants off to get air flow to it and hide from my kids. Everything was heal around day 45 from procedure3. It was pretty even and hard like it should be. Some of my scar was still there but some what better looking. Well day 45 I was back to having sex and feeling better about it i was 7 to 7 1/4 girth depending on excitement but still had the forskin covering the head when limp. Around day 120 shit went south again. I woke up with an even bigger dick. My girth went to 8" and was really squishy when hard. I responded out to loria what I could do about it and no response from them. So here I am with a monster cock that settled down to 7 1/2 hard but squishy as hell and horrible limp looking dick. If I wasn't married my sex life would be over cause I wouldn't show this thing to anyone else. As of now I cant find anyone willing to fix it. Talked three Doctors and they just seem to want to cut it all out. I'm starting to feel as if loria might be the only one who is willing to work on it. Also this was really hard for me to post as I'm calling him out and might have to turn back to him for help. Even though they wouldn't help when I needed it. I really think maybe a circumcision would help or maybe another filler on top of it. Hell I don't know still looking for an answer. I just cant get it all scraped out I really don't think after having a 7 1/2 girth and going back to 5 1/2 girth Is going to give my wife any pleasure. I'm so fucking lost on what to do!

Did he mention anything about dermal fat graft? He mentions it on his website, and it seems he has a good experience with it. It is just clear that Penuma is not something he likes. I wonder then, if he has good experience with AlloDerm and dermal fat graft, why is only Surgimend mentioned here?

Where can I submit questions? Dr. Liu does mention something very interesting concerning ligamentolysis. As you know, methods are used to prevent retraction, but in rare cases I read it is still possible to have retraction.

I was wondering then, why does it happen? And he wrote that it is because of adhesions which can actually be removed. I never read this explanation anywhere before.

Second, I have a question that you might be able to answer: does Dr. Liu actually favor Surgimend over other dermal graft materials? If so, why is that? Because on his website he writes that both AlloDerm and dermal fat graft are good.

It seems he even has experience with Penuma, but he states pretty clearly that he does NOT like it.

Thank you Skeptical_One for the information and you are right...I certainly came to right spot for this topic, I have been reading and likely over-reading all the options for penile enlargement as well as the reviews. I have taken Penuma off the list of options for myself.
Today, I was able to speak with Dr, Liu about Surgimend as well as AlloDerm, and will be looking at both options. I was able to find information and experiences for both on this site; however, still am not sure which will be best for me. I do want to add girth foremost, and length would be great as well, but also look natural and most important: function.

Thank you. What these patients went through was insane. I have officially given up the thought of getting Penuma. Hopefully, there will be much safer options in the future.

For those of you have been thinking about the Penuma surgery, have you had a chance to review Medical Board of CA's amended accusation against Dr. James Elist/Dr. James Jamshid Elist?

I was SHOCKED by what I read.

The document states:

24. Between 2014 and 2018, Respondent employed an individual, Omead M., in his practice. Omead M. was finishing medical school and applying for residencies during this time, but held no license in California, as a physician and surgeon, physician assistant or any other typeof health care provider. Respondent asserts that he did not refer to Omead M. as "doctor" and that he instructed Omead M. not to call himself, "doctor;''

25. According to Respondent, while under his supervision, Omead M. performed medical assistant activities not requiring a license, such as answering emails and writing history and physical findings. Throughout Omead M. 's email correspondences with Respondent's patients, Omead M. states that he is under the supervision of Respondent, and refers to himself and is referred to as "Dr. Omead."


That's crazy.

This document also lists what some patients went through. They reported Dr. Elist to the Medical Board, and this document details how Dr. Elist departed from the standard of care.

Here's an example:

Respondent committed an extreme departure from the standard of care when he failed to take action to have Patient 1 urgently evaluated when, on December 3, 2018, Patient l emailed Respondent photographs clinically suspicious for post-operative infection. Instead, Respondent suggested he apply Neosporin.

Respondent's care and treatment of Patient l further departed from the standard of care as follows:

A. Respondent diagnosed Patient I with PDD without a proper evaluation - specifically, without obtaining or performing a formal psychological evaluation of Patient 1, or making a referral to a mental health professional. Respondent then recommended implant surgery even though the physical examination did not support the chief complaint or assessment.

B. Respondent documented planning a release of the suspensory ligament for Patient 1, even though this procedure is not considered safe.

C. After documenting in Patient 1 's note, "tight suspensory ligament," Respondent then failed to document any description of the suspensory ligament in Patient I's operative notes. Instead, in the operative case dictations it is stated: "suspensory ligament was identified and preserved." Respondent's documentation is inconsistent and inaccurate and appears template when compared to that of his other patients.

D. Respondent had Patient 1 sign a "Release of Claims" in exchange for performing future removal surgery at no cost.

E. Respondent offered Patient 1 a monetary payment in exchange for removing Patient 1 's social media posts regarding the care and treatment he received from Respondent.

The "Prayer" section on the page final page of this document states:
WHEREFORE, Complainant requests that a hearing be held on the matters herein alleged, and that following the hearing, the Medical Board of California issue a decision:
1. Revoking or suspending Physician's and Surgeon's Certificate Number A 35400, issued to Respondent James Jamshid Elist, M.D.;
2. Revoking, suspending or denying approval of Respondent James Jamshid Elist, M.D.'s authority to supervise physician assistants and advanced practice nurses;
3. Ordering Respondent James Jamshid Elist, M.D., to pay the Board the costs of the investigation and enforcement of this case, and if placed on probation, the costs of probation monitoring; and
4. Taking such other and further action as deemed necessary and proper.

You've come to that exact place... it's basically what this site was made for and is today.

Girth is much more attainable than length if it's cosmetic procedures you're looking at.

A simple breakdown involves choosing between surgical or non-surgical.

Surgical options like the Penuma I wouldn't recommend, but dermal graft matrices like Surgimend can be great for those who are looking for permanent, modest gains. If you look at the North American Directory you'll find that Dr. Liu and Dr. Solomon offer these. They are both Board Certified Plastic Surgeons.

Fat Transfer is also an option.

Non-surgical options are essentially dermal filler injections. I wouldn't recommend silicone oil, but there is an abundance of temporary and long-term options to choose from. There are pros & cons to each. Fillers include Hyaluronic Acid (HA), Radiesse, Renuva, Ellanse, and PMMA. If you can, only an hour flight to the Pollock Clinics that offer HA, Adult Circumcisions, Vasectomies, and a whole host of male procedures & services.

Feel free to provide your stats, your goals, and your risk thresholds and I can maybe better point you in the right direction.

Hi! I have been researching and looking at ways to increase my size and girth. I have over-read things and personal accounts which have now made me hesitant to do anything. I was looking at penuma, injections (PRP and fillers) as well as surgical procedures overseas. I was curious if anyone locally has had a procedure? I guess for me, while I would like to increase length as well, increasing girth would be my priority. Am curious to know more first hand information…feel like I need a support group for this stuff and real life accounts. Super hard to know what to do, if anything!!

Forget about this procedure, it is just too risky. Sure some people had a bad experience with filler or dermal graft, but did you ever see a whole community dedicated to the grief of people who got it? No! Look on Reddit and you will find such a Penuma community.
