HI all,

Long time lurker here, the info here (on what is a very personal and not oft-talked about topic) is invaluable. The conversations here prompted me to finally get a ligament cut procedure with Dr. Victor Liu after thinking over it for a decade (scheduled to be done in a few weeks). I'm obviously nervous about getting such a major procedure done and what might happen in case things go wrong, but thought of starting this thread as another real world progress report. I'm going into this procedure with my eyes open and not expecting much. For the record, I'm uncut and at 4.5" erect (I don't do BPEL or anything like that, I just measure the usable length). I've had good partners thus far and enjoyed my sex life, but always wished I could add a bit more. I've tried practically every enlargement device/regimen there is (stuff from monkeybar which I tried for quite a bit, the noose extenders for while, vac extenders, weight hanging, Bib hanger, pegym and other forums, you name it) on and off but got no success. Perhaps it was my lack of "full" commitment or (more likely) simply genetics.

One question I had before I have my procedure done was about Dr. Liu saying he only cuts 90% of the deep suspensory ligament to preserve some stability during sex. I'm curious if other docs just detach the entire ligament (something which Dr. Liu offered if I wanted but did not recommend)? Also wondering if the penis can be elongated without full detachment in the first place. If anyone has any experience on this subject, I'd appreciate your thoughts.

Thanks to all for all the helpful info (especially from Skiman and SouthBay612), I'll do my bit to document my journey as well and will try to be as detailed as possible.

Apologies for the double post

@Texas
You mentioned not using a vac hanger on your aftercare thread if you had a graft done or fillers, correct? I own the bib hanger, and a vac hanger, the vac hanger isn't that tight to be honest...since mainly hanging with the pressure on the glands. What would you recommend as the best method for hanging following this procedure? I also have this still on system leg strap, though I'm wondering if a noose could leave an indentation/gap right behind the glands?

Also heard Skiman mention Malehanger, which seems like it could work.

Any recos? I'll keep searching the forum too...

Just saw a post from Texas which was helpful. Sorry, should have looked harder first!
Clamp style hangers are a must, I can attest to actually having trackable gains from using my "p-hanger" ( its basically a Male hanger / Bib Hanger / etc etc ). Also. while using this style of hanger, i would use a hanger that attaches to ONLY the head area: like a noose ( from Zen Hanger ) and/or a small Clamp Style ( home made ).

I went to Avanti and would recommend. I almost went for Round 3 this week actually but canceled because of scheduling issues. May be for the best since the wife is going to get vaginoplasty later this year tlans they're going to make her "1 finger" tight.

I'm using the Male Hanger. Went for BIB first but the website is 30 years old and unprofessional so I dodged.

Currently doing 3 to 4 sets per day. Started at 20 min sets but due to how much pressure I need for the hanger I was getting cold glans so I increased weight but reduced time. But I'm figuring this out a little more each time I do it. Currently using the BTC between the cheeks method.

@Dthom
It sounds to me like you are moving in the right direction. The 2 hours a day is a great benchmark to hit. The second would be to track your progress every 100hrs of hanging. I think the weight is fine ( at this point ) if you are used to it and handling it well.
BUT...... Lets discuss what type of hanger you are using and next lets discuss having you alternate the types of hangers. Contrary to the popular beliefs out there - Vac hangers produce the least amount gains off all. They do have their place though and should be rotated through. Clamp style hangers are a must, I can attest to actually having trackable gains from using my "p-hanger" ( its basically a Male hanger / Bib Hanger / etc etc ). Also. while using this style of hanger, i would use a hanger that attaches to ONLY the head area: like a noose ( from Zen Hanger ) and/or a small Clamp Style ( home made ). before anyone talks about Noose being bad - Yeah they are, if you use them wrong or to long and without padding. I can use mine for an hour , or a bit more, with out a problem or pain - and if I do get a bit of irritation I stop at once and switch to vac style and either hang or stretch. All of the aforementioned pull differently. Also, each will have different weight capacities. For Example: For me: I am only using head style hanger right now for mine, and vac style and a noose for my stretching. When using a head style ( noose or clamp ) you will find that the weight amount you can is much less. Conversely, when you do Clamp Style ( bib / p-hanger / etc ) you will notice you can do more; but that doesnt mean you should, lol, given the scar i mentioned from the other post.

Hope this helps brother.

The biggest thing to focus on in your aftercare is keeping it elongated to prevent turtling and massaging everything. Into place while it is malleable. Don't use any device that grips the shaft itself for example Bib's hanger. If using a hanger use one that grips right behind the glans. Also I know texas was big on them and after reading some of his stuff I purchased some. Sleeves. I know he's referenced autoxleeve before

Are you saying use the autoxleeve instead of wrapping?

So is it advisable to use sleeves, autoextender devices, etc that is not provided by Avanti? Shouldn’t the noose they provide and massage instructions be sufficient?

How much did you try to stretch after PMMA? I just ordered a vacuum hanger and hoping to gain some length after my 2nd round. I also believe I might have lost some length, probably caused by the significant amount of scar tissue formed.

Not that much honestly. Progress had already slowed to a crawl before pmma, so I was just tired of it.

Hi gents

I wanted to see if anyone else has dealt with having a soft glans/venous leak issue after hanging or manual PE when erect.

I started becoming really dedicated to manual PE around 10 years ago and got into heavy wrapping and hanging along with clamping and some occasional air pumping. My glans was definitely taking a little beating once I was maybe 5 minutes or more into hanging with heavy weight. I used heat wraps to warm up and silicon and bandage wrap under my Bib hanger but my glans still occasionally got a little blue/cold. Looking back obviously that seems dumb to have done, even for 5 minutes multiple times but I also think the pressure of having the weight pulling down on a partially engorged glans, may have damaged the veins as well. All in all, I tried to follow exact instructions from other guys on the sites but maybe I was still doing it wrong.

Prior to that time, I recall my glans would fully engorge with an erection but ever sense, my glans is maybe 50-60% full during an erection. I’ve had one girlfriend say something like, “why doesn’t the tip get as hard unless you really squeeze it?” I didn’t really care that much since it didnt affect having sex but now I realize I may have something going on that could be resolved.

If I really flex down hard I can trap the blood in there and get a full 100% for a few seconds but it’s impossible to keep for longer unless I clamp off the base (like I used to do with clamps which may also be part of the cause).

Either way, I’m stuck with about 50-60% glans max in general and it’s smaller than my shaft which is 4.6 EG so I want to address this before I do any fillers.

Has any here successful used or tried any methods to repair something like this? I’ve read about doing a penis Doppler study, nerve conduction testing , shockwave therapy and possibly surgery that can be performed to ligate veins and close iatrogenic shunts(?).

Any feedback?

Any doctors recommended for someone on the east coast to check it out. I was thinking maybe Morganstern.

Morganstern sent me something that they call PSX2. It's basically a less-fancy version of the Phallosan, with tension applied via a leg strap. Their instructions say to work your way up to wearing the device 8 to 10 hours per day after the procedure. I tried on the device, and it seems to apply surprisingly mild tension, so it'd definitely be tolerable to wear for that length of time--I think the bigger issue would be the inconvenience of having to take it on and off.

I've also recently started hanging using a Bib hanger. I want to ask Dr. Carney's opinion regarding hanging as a means of post-surgical stretching, as it seems like the added tension would be much more effective.
